RE: Bus Stop Locations and Transportation Change Request Form
Dear Parents/Guardians:
As the 2019-2020 school year comes to a close, we are busy preparing for the start of the 2020-2021 school year. For every new school year, students entering Grades 1-12 and who are eligible for transportation are assigned transportation to and from the stop locations closest to their primary home address. You will be notified via email of your assigned stop location in mid-August. Whenever possible, all students are assigned to a group stop location. Students who are eligible for transportation, and will be using the stop closest to their home before and after school, do not need to submit a transportation form.
However, if your child will be in Grade 1-12 and will need transportation to an alternate location, such as to a daily child care provider, or if you will be transporting your child, you will need to complete and submit a Transportation Change Request Form annually. A separate Transportation Change Request Form, if required, should be submitted for each of your students. A request for an alternate stop for child care will only be granted if the stop falls within the daily bus route of the student’s school; a new stop will not be created. Requests must be permanent in nature.
Students are not allowed to take alternate bus transportation for play dates, after-school activities, medical appointments, or employment. Please keep in mind that A.M. stops must be from one location only and P.M. stops must be to one location only. Split stops will not normally be considered; for example, three days to a child care location and two days to home.
All Transportation Change Request Forms must be received prior to Wednesday, July 15. Requests submitted at a later date may face delays in the processing of up to two weeks after the start of the new school year. Forms are only valid for the 2020-2021 school year.
For your convenience, the Transportation Change Request Form for Grades 1-12 is attached. Completed forms can be emailed to adorsey@berlinschools.org, faxed to 860-829-0832, or drop off/ mailed to Berlin Board of Education Office located at 238 Kensington Road, Berlin, CT 06037
